      Located at the intersection of 9th Street and West Broadway. Open 9 am - 9 pm daily, 7 days per week. We accept all insurance plans, Medicare, Medicaid, and Passport. Passport patient need to get a referral from their primary care doctor if they come in before 4 pm. After 4 pm, weekends, and holidays no referral is required.
